Zoom Coupon Terms and Conditions — Consumer

Thank you for your interest in Philips Zoom teeth whitening treatment!

$50 coupon ("Coupon") is redeemable only at participating dental offices (see Dentist Locator on Zoom website below for complete list). Coupon is valid until December 31, 2012, and cannot be redeemed after this date. Coupon may be used towards the purchase of any Philips Zoom in-office teeth whitening services and select Philips Zoom At-home products offered by your participating dentist. Please ask your dentist for eligible products. No other purchase necessary. Void where prohibited. Limit one Coupon per person. Coupon may not be combined with any other offers, coupons, discounts or promotions. Coupon may be used in U.S. locations only.

Philips Zoom may only be dispensed by or on the order of a dental professional. Results may vary. Not all patients are candidates for professional whitening. Please see instructions for use for full details and ask your dentist if you are a good candidate. The whitening process is effective on most discolored teeth. However, darker stains, such as those caused by antibiotics, can be more difficult to whiten. Artificial restorations (crowns, bridges, veneers, bonding, fillings, etc.) will not whiten beyond their original color. X-rays, cleaning, and/or a routine dental exam may be required prior to whitening treatment. Sensitivity during treatment may occur with some patients. If you are concerned about sensitivity or have a history of sensitive teeth, it is recommended that you consult with your Philips Zoom dental professional prior to the procedure to discuss a sensitivity minimization protocol. As with any teeth whitening procedure, Philips Zoom is not recommended for children under 13 years of age, pregnant, or lactating women, or patients currently treated for a serious illness or disorder.
